In Home Case Management

Family Care Network’s (FCN) role and responsibility as In Home Case Managers is to help parents to put parent education theory into practice and to utilize community services for support. The one to one service in the home can be an extension of FCN’s parent education program or a stand-alone service. The Case Manager assists parents with the transition of children back home and, once home, monitors the interaction between the parents or caregivers to assure safety and provide instruction in budgeting, nutrition, appropriate positive behavioral supports and in areas specified in the referral.

The referral from the referring DHS worker should be designated as “in-home case management services” and specify the areas of parenting to be emphasized in the one to one in-home program.
